Our community mental health teams (CMHTs) work with people with severe and enduring mental health issues as well as those with less severe illness who have not responded to provided interventions in primary care services. They offer flexible, responsive and proactive integrated services to individuals with severe and complex mental and behavioural disorders. The service offered to individuals will be provided in the most appropriate setting primarily within the individuals own home and enable individuals in crisis to achieve the highest level of functioning possible in the least restrictive setting; preventing unnecessary hospitalisation and facilitate more rapid discharge from an acute care setting.

CHART(Community Housing and Re-enablement team) works with the trust to ensure homeless and potentially homeless people with mental health problems in Sefton have access to appropriate housing, including emergency accommodation and 24-hour intensively supported accommodation.
