MMD-PROC-025 - Trust Transfer or Discharge to North West Ambulance Service and/or Acute Hospital
This procedure addresses the following:
1. The event of a patient being brought to a walk in centre by emergency ambulance.
2. The event of a patient who needs to transfer from the care of the Trust into the care of North West Ambulance Service (NWAS), and / or an Acute Trust Hospital from a walk in centre due to a medical emergency.
3. The event of a patient in the care of a mental health or learning disability inpatient ward being transferred to an Acute Trust Hospital; this may precede a discharge or the patient may remain in the care of the Trust due to on going Mental Health Act detention status, or continued mental health / risk needs.
The procedure also addresses the event that in this scenario the patient is then transferred back to the ward within Mid Mersey division.
