Who we are
Our community dietitians provide a highly specialised service for adults and children who need nutritional advice and support for a range of conditions, including:
- Malnutrition (undernutrition)
- Failure to thrive (children whose current weight or rate of weight gain is much lower than that of other children of similar age and gender)
- Cancer
- Neurological disorders such as stroke, Parkinson’s disease, epilepsy, Alzheimer’s disease
- Diabetes
- Irritable bowel syndrome
- Food allergies
Did you know that dietitians are the only nutrition professionals to be statutorily regulated and governed by an ethical code, to ensure they always work to the highest standard?
We aim to support positive long-term lifestyle changes to diet, physical activity and behaviour to enable patients to better manage their symptoms, improve control of their disease/condition and improve their quality of life.
We offer appointments in a one-to-one and group setting depending upon the reason for referral and also provide a specialist service those who are tube fed at home.
For health and social care professionals we deliver bespoke training to help promote good nutritional care and general health and wellbeing.
We work with a range of health and social care professionals to provide a patient centred approach to nutritional care. Examples of who we work with include health visitors, school and children’s nurses, speech and language therapists and external agencies such as homecare companies.
Your first appointment will last approximately 20-30 minutes.
It’s helpful to bring your appointment letter, a list of any medications you are taking and contact details for other health professionals who may be involved in your care.
The dietitian will carry out an assessment to find out your individual needs and will review your dietary intake. You may be asked if you are happy to be weighed as this will be helpful to guide on a treatment plan. Your options will be discussed with you and you will be fully involved in agreeing a treatment plan and goals. This may include signposting on to other services.
You may also wish to bring along a partner, relative, friend or carer for support.
We accept referrals from GPs, consultants and other healthcare professionals.
The referral will be triaged and an appointment offered at one of a wide range of venues or at home if required due to being on the housebound register.
We can arrange advocacy services, translation services and support for those with hearing or visual impairments.
