Adult Community Occupational Therapy

Our Community Occupational Therapy Adult Rehabilitation Teams work with patients, and where appropriate, their main carer to support a person to remain in their own home.

Our aim is to maintain or increase someone’s level of independence and ability to carry out daily living tasks. This is usually following illness or injury but also because of the effects of ageing or long-term or life threatening, physical, or medical condition.

We also support people to manage long-term and life threatening health conditions at home to avoid hospital admission. Occupational Therapy interventions can be delivered in the person’s home and local community and are based upon patient identified goals. 

We use the Care Aims Model for assessment and rehabilitation and work closely with other members of the community multidisciplinary team eg physiotherapists, speech and language therapists, dietitians, podiatrists, district nurses and community matrons to ensure a holistic and timely approach to patient care.

Referrals are open to those over 18 years of age who are residents with a Sefton GP. We accept referrals from any health professional who may be involved in the patient’s care. We do not accept referrals which request assessment for equipment only.

Services are Monday to Friday; core times are 8am to 4.30pm with individualised appointments outside these hours, if necessary.

Liverpool COTART team 

The team are based at Liverpool Innovation Park, Babbage House, 2nd Floor Suite, Liverpool L7 9NJ, but patients are not seen here.

We are a city wide service for those over 18 years of age who are residents with a Liverpool GP. Referrals are accepted from health or social care professionals, the patient must consent to the referral. We do not accept referrals which request assessment for equipment only.

Services are available Monday to Friday, 8am to 4.30pm and appointments can be arranged outside these hours if necessary.
