Our team pledge to Children in Care
We promise that you will have access to a Specialist Nurse and their contact details so that you can speak to us/contact us when you need to; We promise to complete your health assessments in a place that you choose and on time; We promise to involve you fully in your health assessments and to give you the opportunity to influence your health plan; We promise to support you to register with local doctors, dentists and opticians; We promise to support you when you want us to and to act as an advocate for you if you need us. We promise to follow through with our actions from your health assessment; be accessible and promise to listen to you.
We are a Specialist team of qualified nurses and administration staff that are trained to work with young people in care. We are part of Mersey Care NHS Trust. We have a responsibility to ensure that your Health Visitor or School Nurse complete your health assessment on time, however the Children In Care Team work primarily with young people who have left year 11. Your school nurse will transfer your health information to our team so that we can continue your care.

If you are aged 16 to 18 years old and new into care your health assessment will be completed by a doctor in a clinic or hospital setting.
We will complete your annual health assessment in your placement or a venue of your choice; we will identify any outstanding health needs and make recommendations in a health action plan to support your health and wellbeing. A summary of your health assessment is shared with your social worker and GP. If any referrals are required these will be included in health action plan.
