This is a multidisciplinary team designed to provide rehabilitation input for adults presenting with long term neurological conditions, such as Multiple Sclerosis, Parkinson’s Disease and Motor Neuro Disease. Support can also be made available for people following a recent acute neurological injury like a stroke or traumatic head injury.

In order to access the service, service users must be registered with a GP in Southport and Formby.

Referrals can be completed by a service user themselves or facilitated by family members, as well from GPs and Heath Care Professionals.

The service provides rehabilitation at home (including nursing and residential homes) or at the Gym (located at Southport Centre for Health and Wellbeing) and work on rehabilitation goals tailored on individual needs with aim to achieve best possible outcome.

The service consists of the following professionals:

  • Occupational Therapists
  • Physiotherapist
  • Speech and Language Therapists
  • Vestibular Consultant.

Referrals are accepted from GPs, consultants or any other appropriate healthcare professional, in addition to self referrals.
