This Multidisciplinary Team (MDT) consists of Advanced Practitioners, Nurses and Therapists.
It supports admission avoidance for frail, older people.
What we do
It works by applying the Bournemouth Criteria / Rockwood frailty scale to identify people who’d benefit from a clinical assessment, risk assessment and care planning and ongoing referral. Patients will have a Comprehensive Geriatric Assessment completed.

- Wednesday: 8.00am to 6.00pm
- Thursday: 8.00am to 6.00pm
- Friday: 8.00am to 6.00pm
