Who we are
In the Southport and Formby Area, referrals can be accepted for adult patients with dietary problems including diabetes, gastrointestinal disorders, obesity with comorbidities, oral nutrition support, nutritional supplement review and home enteral tube feeding.
We don't provide a service for eating disorders, end stage of terminal illness, bariatric surgery, paediatrics or CKD stage 4-5.

- Monday: 8.30am to 4.30pm
- Tuesday: 8.30am to 4.30pm
- Wednesday: 8.30am to 4.30pm
- Thursday: 8.30am to 4.30pm
- Friday: 8.30am to 4.30pm
- Saturday: CLOSED
- Sunday: CLOSED
A referral can be made by your GP or any other Health Care Professional using the Nutrition and Dietetic Department referral form.
Patients will be offered a clinic appointment to be made at their own convenience according to clinic availability. For housebound patients or those in a care home, contact will be made by the team to arrange a domiciliary visit.
The service is provided to patients registered with a Southport and Formby GP.
