What we do

Our team will support clients if they are feeling distressed, and this might include helping the people who care for them, who may be struggling manage.

Who do we support

The intensive support team will support people with learning disabilities who live:

  • In their own home
  • In supported living
  • Residential care
  • In hospital.

Person centred plans

The Intensive Support Team will work with the person to write a person centred plan, which may include a positive behaviour support plan.

They will also work with others involved in their care which may include:

  • Health
  • Social care
  • Carers and families
  • Housing and accommodation
  • Police and probation service.

What can patients expect 

Our team want to ensure the support they give is right. So, they will listen to your needs and wants, and work to try and understand what is causing you to feel distressed.

When the person is feeling better, the team will discharge them back to the Community Learning Disability Team.
