Our Early Supported Discharge (ESD) Service provides ongoing therapy to patients who are discharged from an acute stroke ward following a stroke, who would benefit from further rehabilitation at home or within their place of residence and either live in St Helens or are registered with a St Helens GP.
Referrals are only accepted from an acute stroke unit for patients who have been admitted with a stroke. The ESD team is based at St Helens Hospital and will work with patients for up to six months in their own homes to assist a seamless discharge from hospital. Patients may then be referred on to other community services if there are still identified treatment goals.
