We support people age 65 and over who have mental health problems and people of any age with a diagnosis of dementia. We can also provide assessment and diagnosis for people who are experiencing problems with their memory. You can contact the team directly on 01925 664 041.
We have developed a single electronic referral form for the adults’ mental health Assessment Team. The form is an electronic PDF with editable text boxes so you can type the information directly into the form fields. Once the form is complete, simply press the ‘submit’ button and it will open up an email with the form attached.
Please can you ensure all LLAMS referrals are now sent to the teams electronically via the PDF referral form, rather than via fax.
