Our Community Physiotherapy Service provides short-term domiciliary physiotherapy to patients in St Helens. Patients can be referred to the service by their GP, practice nurse, community nurse, community matron or any other healthcare professional. Please be aware that care for patients being discharged from a hospital inpatient ward is coordinated by Contact Cares.
If you have a patient who is over 18 years of age, is registered housebound and needs physiotherapy you can refer them to our service using our referral form.
