MMD-PROC-005 - End of Life Care for Children Procedure
The community children’s nursing team aims to ensure end of life care for children that is safely and effectively coordinated and facilitated using a collaborative approach between; the specialist palliative care team, community children’s nursing team and Claire house hospice to home service for children aged 0-19 years in their preferred place of care. This is based on guidance from the Together for Short Lives (2013) ‘A core care pathway for children with life limiting and life threatening conditions’
