What our help looks like
We may see you in your home, at school, or at another building near where you live and together, we will come up with a plan to help you and your family to feel happier and safer.
We provide focused assessment and intervention.
We will help you, your family and the people who support you to understand and follow the plan.
We enhance your support from other people by working with your family, to help you to feel safe and secure.
We accept referrals from health and social care professionals for children and young people aged 5 to 18 with varied health needs including learning disabilities, neurodiversity, challenging behaviours and mental health problems.

We work with everyone who supports your child to develop a shared understanding and what we offer together will be tailored to fit your child and your family’s needs.

We are guided by your child’s needs and goals and set a time limit of six months on our involvement. We will regularly review your plan with you and your child so we can notice progress and celebrate achievements together.
