Some people can fall because of medical problems, loss of balance, muscle weakness or loss of confidence.

Falling and fear of falling can have a big impact on your quality of life and general health but research shows that most falls can be prevented.

How we can support you

We work with patients and their carers to offer an individual treatment plan to restore a person’s independence and confidence and reduce hospital admission.

The summary of your assessment will be shared with your GP. You may also be referred to Home Loans, Care and Repair and Lifeline services.

Assessment

We will send you some forms to complete for your assessment which will help us to arrange your personalised plan for you to reduce your falls and risk of fracture in the future.

Your first assessment will take place in one of our community based clinics with one of our specialist falls assessors and will usually last around one hour. You can attend your appointment with a family member or carer with you, and we will discuss why and how you fall, assessing your bone health and fracture risk, reviewing your medication and general health and lifestyle.

At the assessment if you need to use a wheelchair, this can be arranged. If we think that you need equipment or extra exercises for your posture and balance at the assessment, we can arrange a referral for this.

The community clinic allows us to keep waiting times for assessments as short as possible.

Home appointments are available should this be needed, providing you meet our criteria ie. housebound patients. Housebound is defined as someone who cannot leave the house and attend appointments independently (without a carer) or on GP housebound list.

Should you not be able to make your clinic appointment it is very important you let us know as soon as possible so that you can be offered an alternative and we can give your appointment to someone else.

You may be asked to attend future appointments to review the progress of our assessment and personalised plan. This will be done around 12 weeks after your initial assessment.

Referrals

GPs or health care professionals can refer a patient into our service, using the professionals referral form on our website.

Referrals that are accepted from health or social care professionals, will be discussed with you and if you are housebound, we can arrange a home visit.
