Integrated Two Year Review

 

What is the purpose of an integrated two year review?

  • Identify your child’s progress, strengths and the next steps needed to promote positive outcomes in your child’s health, wellbeing, learning and development
  • Identify any support needs as early as possible and offer appropriate intervention and support
  • To allow all children to have equal opportunity to lead a healthy and fulfilling life and reaching their full potential
  • Families are given consistent support and advice and are at the centre of decision making for their child.

How will we do this in Sefton?

Your child will have two separate reviews one completed by the nursery setting in their nursery this is called a progress check and the other completed in the child’s home by the health visiting team, this is called the two year review.

  • All children attending day nursery or pre school will have their progress assessed and monitored from two years of age. This will celebrate your children’s achievements and explore their next steps for learning and development
  • The Healthy Child Programme is a universal health review for young children. Health reviews are designed to monitor the developmen of children and are an opportunity to discuss social and emotional development with parents, as well as linking children to early years services. The two year review will be completed between the ages of two years and two years six months.

If your child is identified as needing further support in their development, you will be offered an integrated review. This means you will be invited to meet with your child’s key worker from their nursery or pre school and a health visitor to discuss the next steps for your child. This meeting will be held at your child’s nursery setting and is called the integrated two year review.

What is an integrated review?

  • Integration means the sharing of information between services and parents or carers
  • By integrating the health and education reviews at this early stage gives a more complete picture of your child
  • Co production between early years, health and the parent/carer will produce a plan with a timeframe of the next steps and interventions that will be put into place for your child. An example of this
  • may be signposting to local help or referral to specialist services, such as speech and language therapy.

How do we share your information

All information gathered at either of the reviews will be shared between agencies when parents have given consent to do so.

Safeguarding children

Agencies have a duty of care to share information and follow local safeguarding procedures where concerns about a child exist.
