About Longmoor House

Mersey Care NHS Foundation Trust (MCFT) and Liverpool University Hospitals NHS Foundation Trust (LUHFT) have worked in partnership to develop a brand-new physical health service.

Longmoor House is a newly refurbished Integrated Care Unit that provides step down capacity from acute trusts for medically optimised patients where it is anticipated that there are ongoing nursing, therapy and reablement needs.

Providing care to patients with reablement needs following a stay in hospital or supporting patients with stepped up care from the community to avoid admission to acute care.  The service will effectively join up the patient’s journey from acute care to home, collaborating and building relationships with LUHFT as well as MCFT community care, Liverpool and Sefton Borough social care teams and third sector providers.
