The District Nursing Service provides a 24 hour nursing service, seven days a week, 365 days a year to vulnerable and housebound people (who are over 18) within their own home or home environment. Each GP practice has an identified District Nursing team. 

Referrals into the service can be from hospital, your GP or another healthcare professional. A district nurse will then perform a health needs assessment with you at home to discuss the help you need.

The service we provide includes:

  • Pressure and wound care and leg ulcer management 
  • Palliative, end of life care, and symptom management 
  • Bladder and bowel management
  • Information, signposting, referrals to other community based services.

We also work in partnership with Willowbrook Hospice, Marie Curie, Queenscourt, Woodsland Hospice and IMPaCT.

Patients need to have clear needs for skilled nursing intervention and referrals will come via your GP or any healthcare professional.
