Community Matrons are Advanced Clinical Practitioners who are experienced in long term conditions and complexity. Their service assists people living in their own home and those living in nursing or residential care settings. They work proactively with patients and carers to coordinate the individual health and social care a patient needs, ensuring people get the right help at the right time.
Community Matrons provide a domiciliary service to support adults who have two or more complex long term conditions who have a very high intensity use of health care. This could include multiple hospital admissions, frequent falls or exacerbations of a health condition.
They aim to optimise patients health and social circumstances within a 12 week period in order to support people to remain at home longer and reduce hospital admissions
Community Matrons support the Enhanced Health in Care Homes Model. This means they also provide clinical review for residents of care homes who are acutely unwell, or who have newly moved to a care home or who have returned to a care home following a hospital admission.
In addition, Community Matrons provide proactive clinical support for care home residents and carers by facilitating regular multidisciplinary reviews in conjunction with care home staff and the GP practice.
