The community specialist Palliative Care teams support patients, their families and carers when the patient has received a palliative diagnosis. The team uses a holistic approach encompassing physical, social, psychological and spiritual wellbeing to help improve their quality of life, and that of their families and carers.
The team work closely with GPs, district nurses and other professionals in a range of settings iincluding hospitals and hospices. The team is available Monday to Friday between the hours of 8.30am to 5pm.
During the weekend, bank holidays or outside of the these hours your district nurse can access the service.
