What can you expect
In mental health, it’s the time when your care moves from children’s mental health services to adult mental health or wellbeing services.
We know that moving into adult support can feel like a big step. You might be feeling excited, nervous, or unsure about what happens next - that’s completely okay.
Your named worker will:
- Talk to you before anything changes
- Help you meet your new adult team
- Listen when you feel worried
- Explain things in simple ways
- Help your family too, if you want.
Important things to remember
You may not always need to be transferred to an adult team if your needs have been met. You should always:
- Feel listened to
- Feel safe
- Feel respected
- Get help that works for you
- Be allowed to ask questions
- Be treated kindly
- You never have to do this on your own.
If you have an EHCP and stay in education this will continue.
Your named worker will meet with you to plan.
This will be in three parts:
Getting ready, steady, go.
It is okay to ask for help.
It is okay to say if you are worried about anything. We will help you.
The support will be for you and help as you move into adult services.
If you require crisis support as an adult you can contact NHS 111 and select option 2.
