Consent to information and treatment
It is important that you have clear information about the the risks, benefits, and alternatives so that you can make an informed decision about your care.
If you feel unsure about giving your consent, please ask us for more information.
As part of your first assessment session, we will discuss treatment options and how they align with your personal goals.
We will send a detailed letter summarising your assessment and agreed plan.
Accessing your health records
You have the right to see information held about you under Data Protection Legislation.
To request your records, please contact your team (details below). We can also support you in going through your records.

Interpreter and translation services
If you need an interpreter for your appointment, please let us know your language needs when you are referred.
We use two NHS approved providers: DA Languages and Language Line.
To request this, leaflet in another language or format, please ask. Information is available in several languages including Urdu, Ukrainian, Polish, Arabic, Romanian, and Punjabi.
Mental Health Act
Being cared for under the Mental Health Act is uncommon, but if it does happen in some circumstances, it’s important that you understand what this means and what your rights are.

Requesting a second opinion
If you would like a second opinion about your diagnosis or treatment, please discuss this with your clinician. Alternatively, contact your team’s central office by email or phone and ask to speak with or write to the Team Manager, who will respond to your request.
We will do our best to arrange a suitable second opinion appointment to meet your needs.
