We are holding an engagement session around discharges from acute hospitals and we would like to hear your views.
The session is aimed at patients and carers in the Liverpool and Sefton area who have had an acute admission in the past six months.
We want to know how this has impacted on you and your family and if you needed additional support.
We would welcome you sharing your experiences to help develop a new service/pathway.
