What we do

Community matrons support patients who have complex long term conditions and currently have a very high intensity use of health care. With the community matrons advanced specialist nursing care, these patients are able to remain at home longer and to have more choice about their health care.

The community matrons work proactively with patients and carers to coordinate the individual health and social care a patient needs, making sure people get the right help at the right time.

Community matrons are based in four localities across South Sefton, Seaforth and Litherland, Bootle, Crosby and Maghull. Each GP practice has a named community matron and we provide care to patients within their own homes.

Access to the service is through a GP, although referrals are considered from all health and social care professionals.

From Monday to Friday, Single Point of Contact will manage referrals and calls for care home referrals.

  • 0151 285 4747 calls are accepted from 8.30am until 4pm, Monday to Friday.

(After 4pm the answer machine message directs callers as appropriate).
